It quite clearly wasn't secured properly for the conditions you encountered during the journey.
Earlier this year I had something similar happen to me. A sheet of something white and about 4 feet square broke off the roof of a van travelling in the opposite direction to me. At the time I thought it was polystyrene and had flown over my car without touching it. So I carried on my journey without stopping.
When I arrived home I discovered a white deposit of plaster at various points on the roof including the rubber seal just above the windscreen. It was then obvious that it had been a sheet of plaster board and it had grazed the roof causing some scratching of the paintwork.
I have a similar sheet of plasterboard in my garage and when I examined it I discovered just how strong it is on edge. If the piece from the van had been an inch or two lower it would certainly have smashed the windscreen and possibly decapitated me. I had less than a second to react to this sheet leaving the van and coming toward me. I would ask others who tie things on to roof racks to be more careful in future.
