Actually it is five years, although they can usually be removed from the licence after four and the points only count for three. However, if you had a 4.5 year old offence since wiped off your licence, you must still legally declare it if asked.

It does. A drink driving conviction will remain on the licence for 11 years. However, it need not be declared for insurance or employment after five years. An endorsement of a licence in itself does not come under the rehabilitation of offenders act and need not be declared. Of course, if a prospective employer or insurer requires sight of the licence, then they will see the endorsement, which might cause trouble.

Note that the five years applies if the conviction involved a fine or community service. If prison is involved, then it rises to 7 or 10 years (or forever), depending on the length of the sentence. It also lasts as long as any disqualification, if that period happens to be longer, although this is rare.

A bit hard to determine which it is, which is why so few convictions are made for this. You might say that asking on the form is requiring the information. In any case, as the person replying has the legal right to deny any conviction, it is not considered that serious.
