This is one reason why I spend most of my time in the first lane. On a fairly busy m-way, I find that lane one is usually pretty empty, with people driving with sensible braking distances. Lane 2 is a little busier, and closer, but lane 3 is just full of cars, with no gaps between them!

I don't think so. It's generally considered good practice to move over, and I do it, but only if the other lane is clear. When I do move over it can quite often leave the left hand lane empty until the other cars join though.
One of the things I do like is where the joining traffic doesn't join straight away. The joiners have a lane (usually making 4 in total) of their own for some distance, so they have penty of time to move over.
It would be nice, but I think there are times when it would be better to use the lane. If the m-way is busy then merging two lanes will create a lot more congestion, and in some cases there may not even be anyone trying to join.
