Not so. I see people accepting responsibility for their actions as important. Not necessarily the endpoint but very important.

As I said, it not about apportioning blame. It's about people accepting responsibility. You may argue that they're the same thing, I happen not to think so. Perhaps what I seek is unattainable because it requires an entire cultural shift but it doesn't stop me wanting it.

If that's all that happens then I agree. But that is not what I am suggesting. Once it has been established that the crash was caused by driver error you need to analyse why? Why did the driver make the decisions he did that led to the crash? Could he have acted differently? Why didn't he? If any of this investigation points to design or engineering flaws in the junction then it should be changed.

I am not, and have never, suggested that people simply "blame" the driver.
