Basically in order to have an operators licence, a CPC (certificate of professional competency) holder has to put their name on it. Usually the CPC holder is the Transport Manager and it is ultimately their responsibility to make sure all legal requirements in regard to the O licence, as well as record keeping, are kept.
What you usually find in a transport office is a file for storing tachos ordered by vehicle registration. Sometimes its nothing more complicated than a stick on a wall for each vehicle registration with all the charts hung on it for that vehicle.
That's OK. Basically, what it breaks down to is the driver keeps all his tachos, no matter what vehicle he has driven, for the current week plus the last of the previous week then hands them into the traffic office who keep the tachos (for 12 months) for each vehicle regardless of who was driving it.
