Were there any arrows or other signs on the approach to the roundabout to indicate which lane was appropriate for going in which direction? If there were it might suggest more who was in the wrong. For example, if the left lane had an arrow pointing left then the truck was certainly wrong to approach in that lane if he wanted to turn right.
Generally, it is always a good idea to check that the left lane is clear before crossing into it, which the lady failed to do. If the lorry was so close as to hit the bumper of her car it may not have seen her indicate, buttuming that she was that rare person who does indicate their intentions early enough for others to see and react on roundabouts.
The lorry also was probably at fault for turning right from a left hand lane as it doesn't sound as though it was big enough to have had any difficulty manoeuvring. Even if he had intended moving to the right or middle lane and was prevented by the lady's car from so doing, he was leaving such a manoeuvre rather late and should have been aware that he was in the wrong lane. Thus, he should have taken more care.
I originally learnt on a motorcycle and still adopt the cautious approach that helped keep me safe. Always buttume that every other driver is out to kill you if they can possibly get away with it. Position your car and indicate in good time what you intend to do and they realise they can't get away with it and you will arrive safely.
