Yes. There was an article in Truck and Driver some time ago about a haulier in Eslove whose drivers were prosecuted on tacho evidence for speeding because they all did 56MPH down the A13 to their base. The nearest legal point they could do 56MPH was several miles from their base.
No and there doesn't need to be although there will be a record of the delivery locations and you'll find that prety much all drivers use the same routes to places which simplifies it somewhat. It is fairly trivial to work out from the speed and distance traces, what route was taken. For example, I can take any random chart that I have and tell you what route I took out of my depot because in one direction there's a mini roundabout at the end of the village that I crawl round and in the other direction it's NSL with no interruptions for about 4 miles.
People don't just break one limit at one specific point, they tend to break the limit over a distance.
