Rather curiously, I saw a small car (Festa size) do a complete 360 degree spin this morning in L3 of three. There was slow traffic in all three lanes, the usual surging forward then coming to a gentle stop, maximum speed around 30mph. Around six cars ahead of me this car just did a complete spin in his lane without touching a thing.
I mentally marked the spot until I got there and found his spin tracks, then tested the road surface myself - expecting to find it covered with ice. It was not even slightly slippy, beyond what you would expect of a damp road. The car gave no impression as it span that it had been caused by faulty brakes - it was a balanced all wheels locked spin look to it.
My best guess is that the driver had perhaps not been paying full attention, had looked up and needlessly panic braked locking all wheels.
Now to the point....
The road surface does not need to be all that bad, for an inexperienced driver to completely loose control of a vehicle once they panic. At no stage in their training are they taught how to properly use the middle pedal, how to brake progressively, how to tranfer the weight effectively, nor how to test the grip of the surface they are driving upon.

They are only learn to press the brake even harder if the car doesn't slow down as fast as they would expect, which is completely the wrong thing to do.
